Orthopedic Injections

Orthopedic injections – also called viscosupplements or hyaluronic acid (HA) knee injections – are used to manage the pain and stiffness of knee osteoarthritis. By supplementing the joint’s synovial fluid, they help restore lubrication and cushioning, and can delay the need for more invasive options. Kinami Health supplies genuine viscosupplements from SYNVISC®, MONOVISC®, ORTHOVISC®, DUROLANE® and CINGAL® to Canadian clinics, orthopaedic surgeons, rheumatologists, sports medicine physicians and licensed professionals, with wholesale pricing and fast delivery.

The orthopedic injection brands available for the Canadian market:

  • SYNVISC® – hylan G-F 20, the best-known viscosupplement in North America, available as SYNVISC® (3-injection course) and SYNVISC-ONE® (single injection).
  • MONOVISC® – a single injection of 88 mg of non-animal cross-linked HA in 4 mL.
  • ORTHOVISC® – a high-molecular-weight, non-animal HA commonly used as a multi-injection course.
  • DUROLANE® – a single injection of 60 mg of non-animal stabilized HA (NASHA) in 3 mL.
  • CINGAL® – a single injection combining 88 mg of cross-linked HA with 18 mg of triamcinolone hexacetonide, a corticosteroid.

How to Choose the Right Orthopedic Injection

Orthopedic injections are not interchangeable. Clinicians usually compare them on four criteria.

  1. Injection protocol – single or multiple injections. Single-injection products (SYNVISC-ONE®, MONOVISC®, DUROLANE®, CINGAL®) deliver the full dose in one visit, which suits patients who cannot easily attend several appointments. Multi-injection protocols (SYNVISC® 3-injection course, ORTHOVISC®) spread the treatment over several weekly visits and allow the clinician to reassess the joint along the way.
  2. HA source. SYNVISC® and SYNVISC-ONE® contain hylan G-F 20, a hyaluronan of avian origin. MONOVISC®, ORTHOVISC®, DUROLANE® and the HA in CINGAL® are non-animal, produced by bacterial fermentation – a relevant point for patients with avian protein sensitivities.
  3. Formulation and molecular weight. Cross-linked or stabilized HA (MONOVISC®, DUROLANE®, CINGAL®, hylan G-F 20) is designed to stay in the joint longer, which is what makes single-injection protocols possible. Linear high-molecular-weight HA (ORTHOVISC®) is typically given as a course.
  4. Need for fast anti-inflammatory relief. CINGAL® is the only combination product in the range: its corticosteroid targets inflammation in the first days after injection, while the hyaluronic acid supports lubrication over the following months.
Brand HA source Protocol Dose per injection
SYNVISC-ONE® Hylan G-F 20 (avian) Single injection 6 mL
SYNVISC® Hylan G-F 20 (avian) 3-injection course 2 mL
MONOVISC® Non-animal, cross-linked Single injection 88 mg / 4 mL
ORTHOVISC® Non-animal, high molecular weight Multi-injection course 2 mL
DUROLANE® Non-animal, stabilized (NASHA) Single injection 60 mg / 3 mL
CINGAL® Non-animal, cross-linked + corticosteroid Single injection 88 mg HA + 18 mg triamcinolone hexacetonide / 4 mL

How Orthopedic Injections Work – Indications & Duration

In a healthy knee, synovial fluid is rich in hyaluronic acid, which makes it viscous and elastic: it lubricates the joint during slow movement and absorbs shock during impact. In osteoarthritis, the concentration and molecular weight of this hyaluronic acid decrease, the fluid loses its protective properties, and pain, stiffness and reduced mobility follow.

Viscosupplementation restores part of these properties by injecting hyaluronic acid directly into the joint. It is mainly indicated for patients with mild to moderate knee osteoarthritis who have not responded adequately to conservative treatment such as simple analgesics, physiotherapy or weight management. Depending on the product and the patient, relief may last up to six months, and treatment can be repeated according to clinical response.

Administering Considerations

Orthopedic injections are for intra-articular use only and must be administered by qualified healthcare professionals trained in joint injection techniques, using strict aseptic technique. Joint effusion should be aspirated before injection where present, and image guidance can improve accuracy. In Canada, who may administer intra-articular injections varies by province and professional college, so practitioners should follow the rules that apply in their own jurisdiction. Product-specific technique is detailed on each brand page.

Side Effects & Safety

Viscosupplements are generally well tolerated. The most common reactions are temporary pain, swelling, warmth or stiffness in the injected joint, usually resolving within a few days. They should not be used in the presence of joint or skin infection at the injection site, or in patients with known hypersensitivity to any component – including avian proteins for hylan-based products. CINGAL® also carries the precautions associated with corticosteroids. In Canada, adverse reactions can be reported to Health Canada.
